GOOD SHEPHERD SERVICES

1.  Your submission of this registration and/or attendance at any workshop is considered an acceptance of the terms of our payment policy.

2.  The person considered responsible for payment is the contact person and/or the trainee.

3.  Effective September 1, 2008: $125.00 per person for a one-day training.  $250.00 per person for a two-day training.  $65.00 per person for a half-day training. An invoice is generated upon confirmation of your registration.

4.  To register for a workshop, register online or print a copy of the registration form, fill out all the required fields and fax it to us at 212-675-2260. Our workshops are on a first-come, first-served basis.  You are only considered registered once you receive confirmation from us. If you do not receive confirmation within seven business days, please call us at 212-242-2293. 

5. Submit your registration immediately.  Follow with your payment as soon as possible. As your registration and payment hold your slot we expect payment two weeks in advance of each workshop.  Checks should include the workshop number & name of participant and be made payable to, “Good Shepherd Services / HSW.”

6.  Refunds will not be given.

7.  Credit will be given for a workshop only if a cancellation is received at least 3 business days before the first day of the workshop. Credit must be used within 6 months. 

8.  Substitutions are accepted. You must call to notify us of the name change. If a person’s name is not on our list, the substitution will not be accepted.

9.  Check in begins at 9:00 AM. The workshop begins promptly at 9:30 AM. No one will be admitted after 10:00 AM. As a reminder, there will be no refunds or credit given.

10.  Full payment is due if the participant attends any part of the training. If a workshop is given in more than one session, participants are expected to attend all sessions.

Coffee service is provided and lunch is on your own.
